• Personal health history, if possible in chronological order.
• History of childhood illness and ensuing complications.
• Photo from you as a child and/or baby.
• Your dental history and possible ensuing complications.
• History of all vaccinations administered to you and possible ensuing complications.
• History of all hospital stays and operations.
• A list of medical drugs you took over a long periode of time or are taking now.
• Please think about major emotional trauma.
• Please think about when your chief complaints started and what might have caused them.
